Clinical Pharmacology & Mechanism of Action
PT-141 (Bremelanotide) is a synthetic cyclic heptapeptide analog of alpha-melanocyte-stimulating hormone (α-MSH). It was originally developed from Melanotan II as a sunless tanning agent, but clinical trials revealed a much more profound effect: it is an extremely potent aphrodisiac and treatment for sexual dysfunction in both men and women.
Unlike sildenafil (Viagra) or tadalafil (Cialis), which operate strictly on the vascular system to increase blood flow to the genitals (PDE5 inhibitors), PT-141 acts entirely on the central nervous system. It binds primarily to the melanocortin-4 receptor (MC4R) in the hypothalamus.
By activating these neural pathways, PT-141 induces spontaneous sexual arousal, desire, and strong erections/engorgement by stimulating the brain's sexual center directly .
This makes it effective for psychogenic erectile dysfunction (where PDE5 inhibitors fail) and Hypoactive Sexual Desire Disorder (HSDD) in women, for which it is FDA approved under the brand name Vyleesi.
Pharmacokinetics & Timing
Because it acts on the central nervous system rather than the vascular system, PT-141 has a highly unpredictable pharmacokinetic profile regarding onset of action.
- Onset of Action: 2 to 6 hours. (Unlike Viagra which works in 30 minutes, PT-141 requires significant advanced planning. The arousal effects often peak around the 4-6 hour mark).
- Duration: The aphrodisiac effects are known to linger for 24 to 72 hours in many subjects.
Standard Research Dosing Protocols
PT-141 is infamous for its dose-dependent side effect: intense nausea. Finding the minimum effective dose is critical.
- Starting Dose (Assessment): 0.5mg to 1.0mg. (Researchers strongly recommend starting here to assess the nausea response).
- Standard Effective Dose: 1.5mg to 1.75mg.
- Maximum Clinical Dose: 1.75mg per 24-hour period.
- Frequency: Do not use more than 8 times per month, and leave at least 24 hours between doses to prevent melanocortin receptor desensitization.
Reconstitution Math & Syringe Guide
PT-141 is typically sold in 10mg vials. Because the clinical dose is 1.75mg, extracting accurate doses from a highly concentrated vial requires precise math.
Step-by-Step Reconstitution (10mg Vial)
- Swab the rubber stoppers of your 10mg PT-141 vial and Bacteriostatic Water.
- Using a sterile mixing syringe, draw exactly 2mL of Bacteriostatic Water.
- Inject the water slowly down the inside wall of the PT-141 vial.
- Swirl gently until the peptide is fully dissolved.
The Math for a Maximum Dose (1.75mg):
- 10mg total in the vial.
- You added 2mL of water.
- Therefore, the concentration is 5mg per 1mL.
- You need a 1.75mg dose. Divide the dose by the concentration: 1.75 ÷ 5 = 0.35mL.
- 0.35mL = 35 units on a U-100 syringe.
(For a safer 1.0mg assessment dose: 1.0 ÷ 5 = 0.20mL, which is 20 units).
Common Mistakes & Troubleshooting
- Jumping to the Maximum Dose: Injecting 1.75mg or 2.0mg for the first time almost guarantees severe, debilitating nausea that ruins any aphrodisiac effect. Start at 1.0mg or lower.
- Timing the Dose Poorly: Because the onset takes 4-6 hours, researchers who inject 30 minutes prior to intended activity assume the peptide is "fake," only to experience the effects hours later while trying to sleep.
- Stacking with PDE5 Inhibitors Cautiously: While some researchers stack PT-141 with low-dose Cialis for a synergistic effect, this can cause profound drops in blood pressure and intense flushing.
