Tirzepatide Research Profile

FDA Approved

Tirzepatide is a first-in-class dual glucose-dependent insulinotropic polypeptide (GIP) and GLP-1 receptor agonist. The synergistic action of both receptors produces significantly greater weight loss than pure GLP-1 agonism alone.

Mechanism of Action & Biochemical Context

Tirzepatide is a first-in-class dual glucose-dependent insulinotropic polypeptide (GIP) and GLP-1 receptor agonist. The synergistic action of both receptors produces significantly greater weight loss than pure GLP-1 agonism alone.

Storage, Reconstitution & Stability Guidelines

Prior to reconstitution, lyophilized Tirzepatide powder should be stored away from light between 2°C and 8°C for short-term access, or -20°C for long-term preservation. Once reconstituted with Bacteriostatic Water (0.9% benzyl alcohol preservative), maintain strict refrigeration between 2°C and 8°C. Do not freeze liquid solution.

Frequently Asked Questions

Add 2mL of Bacteriostatic Water to the 10mg vial. This gives a concentration of 5mg/mL. For a 2.5mg dose, draw 0.5mL which is exactly 50 units on a U-100 insulin syringe. The calculator above will handle this math automatically if you enter your vial and water amounts.

Tirzepatide consistently produces greater weight loss. In the SURMOUNT clinical trials, subjects on 15mg Tirzepatide lost 20-22% of body weight on average, compared to 14-17% in Semaglutide STEP trials. The difference comes from GIP receptor agonism, which increases energy expenditure and improves insulin sensitivity in adipose tissue, on top of standard GLP-1 appetite suppression.

The standard clinical escalation is: 2.5mg (Weeks 1-4) → 5.0mg (Weeks 5-8) → 7.5mg (Weeks 9-12) → 10.0mg (Weeks 13-16) → 12.5mg (Weeks 17-20) → 15.0mg (Week 21+). Each step lasts a minimum of 4 weeks. Escalation should only occur if the current dose is well tolerated without severe nausea or vomiting.

There is no validated, direct cross-taper protocol. Most clinical researchers do not start a Semaglutide-experienced subject on the maximum 15mg Tirzepatide dose. A conservative approach is to restart at 5.0mg or 7.5mg (a mid-tier dose) and allow 4 weeks for re-acclimatization to the dual agonism before escalating further.

The milligram numbers are not directly comparable. Tirzepatide (2.5mg starting dose) vs. Semaglutide (0.25mg starting dose) - Tirzepatide appears "10x higher" in mg, but the two molecules have completely different affinities for their receptors. You cannot compare potency by looking at milligram numbers alone.

When stored refrigerated at 2-8°C with Bacteriostatic Water, reconstituted Tirzepatide remains stable for approximately 28 days. It should never be frozen after mixing. Dry, lyophilized vials can be stored in the freezer for 1-2 years.

Like all GLP-1 class drugs, Tirzepatide does not discriminate between fat and muscle when creating a large caloric deficit. Studies suggest that approximately 25-40% of weight lost on GLP-1 drugs can be lean muscle mass. This makes a high-protein diet (1.2g+ per kg bodyweight) and consistent resistance training absolutely critical during any Tirzepatide research protocol.
